CHC is proud to be a partner in Closing the Gap, an important infant mortality reduction initiative focused on two clusters of community areas in Chicago. Funded by the federal government in only four states in the country and coordinated by the Illinois Department of Human Services, this project works to decrease the rate of infant death from prematurity and Sudden Infant Death Syndrome (SIDS). CHC is collaborating in this work with the lead agency, the Illinois Maternal and Child Health Coalition, and the Illinois Chapter of the March of Dimes and the SIDS Foundation of Illinois. One aspect of the project is training front-line service providers and developing a cadre of peer educators who can engage with community members to provide information and support around health promotion and disease prevention. Chicago Health Connection is pleased to play a role in the Closing the Gap Project in Illinois by providing training and technical assistance to prevent prematurity and SIDS in the highest-risk community areas in the state.
