PC Placements & Collaborations
Since our founding in 1986, Chicago Health Connection (CHC) has
been collaborating with community agencies to provide community-based
outreach and health promotion. We recognize the strengths
of the communities we work with and respect the voices of the people
who live there. Before we work with an agency, we meet with key
personnel to determine that a community need exists and that the
program responds to these needs.
We promote collaborative relationships with a variety of health and social service agencies. Our partnerships with agencies that serve low-income women increase the impact of our peer counselors' work. Since 2001, we have developed five new breastfeeding peer counselor positions in direct-service agencies around Chicago.
CHC has trained staff or placed breastfeeding peer counselors at a variety of community agencies including:

- Firman Community Services
- Access Community Health Network
- Christopher House
- Marillac Social Center
- University of Chicago Friend Family Health Center
- Lawndale Christian Health Center
- Norwegian-American Hospital
- TCA Health, Inc.
- Chicago Urban League
- Mercy Family Health Center
- Maternal Infant Health Outreach Workers (MIHOW)
- Women’s Treatment Center
We are always looking for community-based agencies that would like
to train women from their community to serve as breastfeeding peer
counselors. We are especially interested in collaborating
with agencies to develop new breastfeeding peer counselor programs.
We seek to work with agencies that value the knowledge and personal
experience that these women bring to their role as peer counselors,
and are also committed to investing the personnel and financial
resources to develop and sustain a peer counselor position.
